Manhattan Rhinoplasty

Rhinoplasty and Nose Job are terms broadly applied to any Manhattan rhinoplasty operation, which affects the appearance of the nose. Often times, rhinoplasty can be combined with septoplasty to enhance nasal breathing. Septoplasty or correction of a deviated septum addresses nasal blockage due to misalignment of the midline wall in the nose. Septoplasty alone typically does not affect the shape of the nose.

Surgical outcome of any nose job operation depends on the quality and position of the nose cartilages, the size and shape of the nasal bones, and the quality and thickness of the nasal skin. Ethnic considerations also play a large role in the surgical techniques and surgery results.

Rhinoplasty in New York City and Nose Jobs in New York City are typically performed under general anesthesia in the operating room. Patients go home the same day. We do not use nasal packing, which can be uncomfortable and even painful. Patients notice nasal congestion for about a week, and may experience a mild bruise for ten days. Depending on the steps that were taken during the procedure, patients may have some tape or plastic cast on the nose. In either case, the patient may shower the next day. If there are any external sutures, they are removed in five to six days along with the cast.

Dr Khosh is uniquely qualified to perform Rhinoplasty in New York City and Septo-Rhinoplasty in New York City based on his experience, and double board certification in Facial Plastic Surgery and Otolaryngology. He has intimate knowledge of both the internal anatomy and external topography of the nose, which enables him to provide cosmetic improvement of the nose without sacrificing breathing ability. He is an expert in all approaches to the nose, and routinely treats complicated New York City Revision Rhinoplasty patients.

In the following sections, Dr. Khosh will present some of his patients who have undergone rhinoplasty for various reasons. He will briefly discuss the steps involved in those operations.

Humped or Hooked Nose Rhinoplasty / Nose Jobs in New York City:

Humped or Hooked Nose Rhinoplasty / Manhattan Rhinoplasty

A large hump or hood in the nose draws attention to the nose, whereas, a well proportioned nose imparts harmony and congruity to the face. Nasal humps are due to overgrowth of the nasal bones and cartilages. In addressing this problem, the nasal bones are shaved down as is the cartilage, and the bridge of the nose is narrowed to create a natural appearing result. When treating a humped nose, Dr Khosh also addresses any deviation of the septum in order to avert breathing difficulties following surgery.

Rhinoplasty / Nose Jobs in New York City to improve nasal tip appearance:

A round or large nasal tip can impart a coarse appearance to the entire face. Nasal tip appearance is due to position and shape of the lower nasal cartilages, as well as thickness of the nasal skin. Tip rhinoplasty involves shaving and/or bending of the lower nasal cartilages with sutures. Occasionally small cartilage grafts are utilized to enhance nasal tip definition. No matter which technique is utilized, natural appearing results are paramount.

Rhinoplasty / Nose Jobs in New York City for the crooked nose:

Nasal trauma and nasal fractures can lead to a crooked appearing nose. The nasal bones or nasal cartilages or both may be misaligned causing crookedness in the upper nose, the middle part of the nose, or the nasal tip. Achieving a straight nose may require breaking and re-aligning the nasal bones, repositioning the cartilages, or adding cartilage grafts. These procedures can be combined with techniques to enhance the tip, or reducing a nasal hump.

African American Rhinoplasty / Nose Jobs in New York City:

The anatomy of the African American (platy Rhine) nose is usually different from the Caucasian nose, in that the dorsum is wider and lower, the skin is thicker, the cartilages are softer, and the nostril base is more flared. With African American rhinoplasty / nose jobs in New York City, Dr Khosh aims to enhance and refine the ethnic features of each patient. Maneuvers such as narrowing the nostril base, adding cartilage grafts, and dorsum narrowing are employed. Effective communication and the use of computer imaging allows Dr Khosh to clearly understand his patient's desires, and to set natural and realistic goals.

Asian Rhinoplasty / Nose Jobs in New York City:

In contrast to Caucasian patients, Asian patients often desire increased height of the nasal dorsum and improved nasal tip projection. Maneuvers such as placement of dorsum implants and nasal tip grafts are very effective in reaching these patient goals. Since most Asian patients have adequately thick skin, implants and grafts work very well. Computer imaging during the consultation phase allows Dr Khosh to give patients a reasonable expectation of what surgery will achieve.

Rhinoplasty / Nose Jobs in New York City for cleft lip patients:

Patients with congenital cleft lips also experience misalignment of nasal cartilages, and nasal tip deformities. Cleft lip abnormalities are usually addressed shortly after birth. Secondary nasal deformities in these patients, however, are usually not treated until the patient is older. The aim of surgery is to create symmetry and restore a normal degree of nasal tip projection. Rhinoplasty in these patients helps to erase some of the stigmata of a congenital abnormality.

Rhinoplasty / Nose Jobs in New York City for collapsed nose (Saddle nose deformity):

The nasal septum (midline wall inside the nose) has a critical role in stabilizing the middle third of the nose. When the septum is damaged or traumatized significantly, the middle of the nose may collapse giving the nose a "saddle" appearance. The middle vault collapse causes an additional deformity manifested by severe upward rotation of the nasal tip. Correction of saddle nose deformities requires finely honed surgical skills and utilization of various cartilage or bone grafts form other parts of the body including the ear, the rib, or the outer layer of the skull. Dr Khosh has masterfully treated large numbers of patients with this unusual and devastating problem.

Rhinoplasty / Nose Jobs in New York City for the traumatized nose:

Nasal fractures can cause both external and internal deformities, which would affect the appearance and the function of the nose. In treating a traumatized nose, our goal is to restore the appearance of the nose by realigning the displaced nasal bones. When breathing has been compromised, septoplasty is performed to alleviate nasal obstruction. Within the first two weeks of injury, the less severe fractures can be treated as an office procedure under local anesthesia.

Rhinoplasty / Nose Jobs in New York City for the aging nose:

As one ages, the ligament attachments between nasal cartilages weaken, and the nose tip begins to sag. A sagging nasal tip can be more than an aesthetic concern. Non Surgical Rhinoplasty / Nose Jobs in New York City:

The current availability of injectible fillers allows treatment of certain types of nasal deformities without surgery. This comprises a narrow group of patient who desire refinement of the nasal profile, through elevation of depressed or low areas in the bridge of the nose. Injection of fillers such as Restylane, Juvederm, or Radiesse causes elevation of a depressed regions of the nasal bridge, thereby enhancing the profile. Fillers should not be injected into the nasal tip, and the results are temporary lasting six to twelve months. Although the FDA has approved use of these fillers in the cheek skin, their use in the nose remains off label.

Juvederm was used in this patient to improve the apparent depression of her nose, which occurred following rhinoplasty over ten years previously.
